ACH Authorization Agreement

You authorize the Program to initiate debit entries for the amount(s) specified to the checking or savings account(s) at the financial institution(s) you indicated, and said financial institution(s) to withdraw the specified amount(s) from such checking or savings account(s).

You represent and warrant that you are legally authorized to use the checking or savings account(s) that you have specified on the Service. You agree that this authorization will remain in full force and effect until the Program receives notification from you of its termination. You represent and warrant that the specified checking or savings account(s) are personal account(s) and not business/corporate account(s).
